Services Available on Referral:- Occupational Therapy -- for assessments of function and seating for purchasing a wheelchair
- Physiotherapy -- for assessment of and setting up a personalized exercise program
- Speech Therapy -- individualized speech therapy, swallowing assessments, and swallowing management

- Geriatric Assessment
- Social Work
- Psychogeriatric Assessment

- Have health care needs that cannot be met with any combination of caregiving, care in your home or care in your community
- Have health care needs that can be met in a long term care home
