Service Description:	Operates as a community-based organization offering primary health care and health promotion programs in areas within the district Offers a Diabetes Mobile Health Team consisting of a Nurse Practitioner, Registered Nurse, Dietitian, Foot Care Nurse and Community Health Worker Travels to small communities to provide diabetes education and support to individuals at risk of or living with diabetes
