Service Description: Provides a variety of rehabilitation services, mental health and addiction services, and long-term care services

Supports for people living with prediabetes or diabetes
  • Provides the knowledge and skills for people with diabetes to take control of their health
  • Designated as an Assistive Devices Program registered for the provision of insulin infusion pump training in Northwestern Ontario

Pediatric Diabetes
  • Offers education and clinic services to families of children and youth with diabetes
  • Provides necessary survival skills to allow the patient to manage their diabetes at home
  • Provides ongoing medical care and support
  • Offers educational and social events, including support groups

Pregnancy Program
  • Provides education in meal planning, blood glucose monitoring, use of insulin pens, and recording results
  • Offers preconception counseling to ensure good control of diabetes before pregnancy
  • Provides follow up during the pregnancy as well as post-partum, providing further support as required

Type 1 and Type 2 Diabetes in Adults
  • Provides education and support as well as access to the tools and resources to help clients self-manage

Prediabetes Program
  • Teaches individuals who are at risk for diabetes the skills to help them to prevent or delay diabetes
  • Provides information in a group setting on topics such as activity, cholesterol, weight management, and healthy eating

Application:
  • Open to referrals from any source including self-referrals

  • Call to schedule an appointment for initial intake

  • Bring glucose monitor, blood sugar records and list of current medications to each appointment
