For after-hours needs, patients can either call the Telephone Health Advisory Service at 1-866-553-7205 or go directly to the Nipigon District Hospital, where they will be assessed by a registered nurse in the Emergency department.

Service Description: Family Health Team includes: Family Physician, Registered Nurses, Registered Practical Nurse, Social Worker, Registered Dietitian, Addiction Program Care Coordinator, Administration and other professionals
  • Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy
  • The vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues


  • Offers timely access to family and community primary health care
  • Provides access to other health care professionals within and outside of the Family Health Team
  • Coordinates documentation if patient receives care from a member of the team
  • Administers various health programs
  • Offers primary health care including but not limited to:
    • Cancer screening
    • Chronic disease education and prevention
    • Complex foot care
    • Diabetes and hypertension monitoring, screening, and education
    • Disease management
    • Health promotion
    • Holter/loop recorder monitoring
    • Immunizations
    • Mental health and addictions services
    • Outreach services
    Telephone Health Advisory Service
    • Provides access to a registered nurse that provides advice about urgent health care concerns
    • Offers the opportunity to have family doctor receive a written summary of the call

    Fees: Most services covered by OHIP

    • No referral required for most services
