Service Description:

Assists individuals who are living with the effects of a brain injury attain their maximum potential as they continue living, loving, and doing

Helps individuals to participate in meaningful life roles at, or as close as possible to, the level they did prior to their acquired brain injury
Provides flexible services to meet the diverse needs of individuals who require rehabilitation and support services
Provides referrals to the residential services available in Thunder Bay
Provides Community Rehabilitation and Outreach Services in Thunder Bay, Kenora, Fort Frances and Dryden

Offers Assisted Living Services in the following locations:

Application:

Referrals by family, health professional, physician, lawyer, or self-referral
Intake process includes completed and signed "Application for Service" (AFS) form which can be found on the website
AFS form can be signed by substitute decision maker

Eligibility / Target Population:

Individuals 16 years of age and older who have an acquired brain injury
