Service Description: Provides a multi-site, multi-function health care system serving the residents of the Rainy River District

Provides rent-geared-to-income (RGI) supportive housing in landlord/tenant units for individuals with challenges
  • Provides assistance in securing rent geared to income (RGI) housing units
  • Operates under the Residential Tenancy Act
  • Focuses on independent living with minimal assistance
  • Provides 24-hour on-site support staff
Provides the following services as required:
  • Advocacy with other programs and services
  • Assessment and monitoring
  • Case management
  • Life skills training
  • Linking and referrals to other services
  • Medication management and monitoring
  • Service coordination
  • Supportive counselling

Fees: Service - Rent (geared to income)
Application:
  • Referrals from any source including self-referral

  • Intake assessment required

  • Housing application must be completed
Eligibility / Target Population:
  • Low to modest income individuals 18 years of age and older and

  • Are living with mental illness, mental health issues, developmental challenges, physical challenges, and/or aging challenges
