Service Description:

Provides a multi-site, multi-function health care system serving the residents of the Rainy River District
Provides rent-geared-to-income (RGI) supportive housing in landlord/tenant units for individuals with challenges- Provides assistance in securing rent geared to income (RGI) housing units
- Operates under the Residential Tenancy Act
- Focuses on independent living with minimal assistance
- Provides 24-hour on-site support staff
Provides the following services as required:- Advocacy with other programs and services
- Assessment and monitoring
- Case management
- Life skills training
- Linking and referrals to other services
- Medication management and monitoring
- Service coordination
- Supportive counselling

Eligibility / Target Population:
|
- Low to modest income individuals 18 years of age and older and
- Are living with mental illness, mental health issues, developmental challenges, physical challenges, and/or aging challenges
