Service Description:

The Red Lake Family Health Team includes: Family Physicians, a Nurse Practitioner, Registered Nurses, Dietitian, Social Workers and Administration- Family Health Teams allow physicians and other
allied health providers to work as a team to focus on keeping patients healthyThe vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues
Offers health services and a variety of health programsProvides access to other health care professionals within and outside of the Family Health TeamCoordinates documentation if patient receives care from a member of the teamAdministers the following programs and services:- Chronic Obstructive Pulmonary Disease (COPD) or Asthma Program
- Diabetes Self-Management Program
- 18-Month Well-Baby Visits
- Hypertension Program
- INR Management Program
- Memory Clinic
Also provides access to services through the following community partners:- Ontario Health atHome - North West Area
- Northwestern Health Unit
- Community Counselling and Addiction Services
- CAMH Centre for Addiction and Mental Health
