Service Description:
|
Family Health Team includes: Nurse Practitioners, Registered Nurse, Registered Social Worker, Health Educator, and Administration who work closely with Physicians in Geraldton Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthyThe vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues
Medical care team focused on family and community health- Offers timely access to primary care through Advanced Access (same-day bookings)
- Provides access to other health care professionals within and outside of the Family Health Team
- Coordinates documentation if patient receives care from a member of the team
- Administers various health programs
- Offers a walk-in medical clinic for urgent, primary health care concerns
Offers primary health care services including but not limited to:- Chronic disease education and prevention
- Disease management
- Health promotion
- Palliative care
- Rehabilitation

Walk-in Clinic
Present in-person between 8:30 am-2:30 pm to sign up and receive approximate time to return to see health provider - appointments are given on a first come-first served basis

- Registered clients of the Family Health Team
- Must have a valid Ontario health (OHIP) card
