Service Description: Provides a wide range of innovative services and supports to people who are experiencing mental illness and their families

  • Responsible for providing a variety of specialized community geriatric services
  • Promotes issues surrounding geriatric mental health
  • Provides community outreach and presentations
  • Program services include:
    • Cognitive screening and assessment
    • Geriatric counselling or therapy
    • Care and treatment planning
    • Follow up, referral and advocacy services
    • In-home support and education to caregivers
    Geriatric Psychiatry

    Provides assessments and case consultations via videoconferencing with the Toronto Baycrest Centre for Geriatric Care

    Psychogeriatric Resource Program

    Provides education and support in long term care facilities to mental health clients and staff

    Application: Intake assessment required
    Eligibility / Target Population:
    • Individuals 60 years of age and older

    • Must have a diagnosis of mental illness, Alzheimer's Disease or a related dementia
